I believe in you. Its all about habit. Id suggest making it the easiest possible activity. For example, having equipment at home allowed me to keep consistancy and not skip days and eventually quit.
Be cognizant of your thoughts and emotions and analyze them. If you are tired at the end of the day and really wanna skip the gym, analyze what is making you feel that way and do your best to remove that hinderance.
To form an habit, make it easy.
