Because the entire world around him is fake. He's not really alive. He's in a timeline 'purgatory' equivalent. From here he can't die, but will suffer increasing uneasiness and pain, while literal demons in the form of other people try to trick him into falling into ever worse shadow realities from which there is no escape. He can avoid it but he's walking a tightrope and the prevailing current is increasingly trying to push him over the edge. I know that he can feel it rising, starting with a background uneasiness the second he arrived, but inexorably rising like a dull pain, the oncoming distant infrasound of a distant approaching train. The demons are held off for a time, but will gradually given more influence and they are clever. He must realise his situation and steel himself against it before it's too late.
